Coronavirus (COVID-19): Maintaining a Safe and Effective Care Environment

A safe and effective care environment is our highest priority, and Westchester Medical Center Health Network (WMCHealth) hospitals and medical practices have undertaken necessary steps to protect our patients and workforce as the novel coronavirus (COVID-19) situation evolves.

Visitation

Visitation for Schervier Pavilion is aligned with current NYS DOH and CMS Visitation Guidance.  There are no restrictions to visitation however, visitors who are COVID-19 positive, currently meet the criteria for quarantine and/or have symptoms of COVID-19 will not be permitted to enter the facility.

Schervier Pavilion is a 120-bed, skilled nursing facility dedicated to the highest standard of healthcare excellence. At Schervier Pavilion we are dedicated to meeting the needs of our residents in an atmosphere of caring and warmth.

A Safe Place to Call Home

Currently, Schervier Pavilion received a five-star rating from the Centers for Medicare & Medicaid Services (CMS), which rates nursing homes nationwide that participate in Medicare or Medicaid. Nursing Home Compare, a resource on the Medicare website, gives each facility a rating between one and five stars. The ratings come from a combination of health inspections, staffing and quality of resident care measures at each specific nursing facility. CMS calculates a star rating for each of these three categories, along with an overall facility rating, with a five-star rating indicating a facility’s exemplary care and physical environment.

At Schervier Pavilion, our goal is to help each resident maintain the highest level of daily independence possible. Residents participate in a broad range of therapeutic and social activities designed to enhance their physical and mental capabilities. A team of highly qualified and dedicated professionals provides all services. Located on a 27-acre campus, Schervier Pavilion residents have access to a complete continuum of care. Among our many services you will find:

Specialized Care in a Lovely Setting

Residents of Schervier Pavilion enjoy lovely accommodations, around-the-clock, highly trained licensed staff, therapeutic and restorative nursing, comfortable lounges and activity rooms, and climate-controlled heating and cooling for comfort. Under the direction of a certified recreation therapist, residents are offered a variety of social, recreational and cultural opportunities designed to provide mental and physical stimulation. At Schervier Pavilion we not only care for residents, we care about them.

Therapeutic Recreation

The Therapeutic Recreation Department at Schervier Pavilion improves the quality of life for residents by offering social, intellectual, physical and emotional stimulation while encouraging independence.

The department provides a variety of activities, geared to residents' abilities and interests, seven days a week including evenings, weekends and holidays:

Residents also participate in their community by enjoying lunch outings, shopping trips and trips to  farm markets. A calendar that outlines the daily planned activities is distributed to residents at the beginning of the month.

Affordability

Application for Medicaid benefits may be made once a resident's funds and/or assets have reached the level specified by the Medicaid program. Assistance in applying for these benefits is provided by the admissions coordinator. The resident making the application and/or their responsible representative needs to provide financial records and other pertinent information as requested to facilitate the application process.

Hear From Our Residents 

Testimonial: Joseph Saye

Joseph Saye, 65, is a former Greenwood Lake resident who spent most of his life on the road as a truck driver and mechanic. Ten years ago he started to experience symptoms of lymphedema in his legs which led him to seek treatment at St. Anthony Community Hospital. He was in and out of the hospital over the course of the next few years due to complications from lymphedema, and he realized that it would be healthier for him to move to a place where he could have his condition consistently monitored.

“My mother was actually a resident at Schervier Pavilion, and I was so impressed with the care she received. When I needed to find a facility for my 24-hour monitoring, I knew I wanted Schervier Pavilion. I have been to a few other short-term care facilities in the area, and they do not compete with the quality of care offered at Schervier Pavilion,” said Saye.

Saye has been a resident at Schervier Pavilion for over a year and receives physical and occupational therapy along with 24-hour monitoring for his various conditions. He feels completely safe and is extremely thankful that he has on-going care. He points to a specific incident that solidified his confidence in the Schervier Pavilion staff.

“My nurse, Megan, was about to administer my medication to me, when she realized I was not doing well. She immediately recognized my symptoms and started treating me for a heart attack. I went right to St. Anthony Community Hospital. She was so fast in responding to what was going on with me; she saved my life. It was nice to have the hospital [St. Anthony Community Hospital] right there on the same campus, so they could take care of me right away. St. Anthony Community Hospital is an excellent hospital, and the team there is competent and kind,” said Saye.

Saye is also impressed by the personal attention the staff at Schervier Pavilion provides the residents. “Everyone makes a connection with you. My nursing aide, Grace goes out of her way to make sure I am taken care of. Even on the days she isn’t responsible for my floor, she makes a point to come in and say hello,” said Saye.

Going the extra mile is a trait seen across the Schervier Pavilion community, including many enriching events and activities. Saye is an auto enthusiast who loves classic and fast cars. Upon hearing about his interest in cars, Terry Cote’, Director of Social Service at Schervier Pavilion worked with Saye to set up a Car Club for the residents. The group meets once a month to talk about cars, look at photos of some of their favorites and watch movies. It is something Saye looks forward to each month.

Saye is very satisfied with his choice to live at Schervier Pavilion, and recommends it to anyone looking for a long-term care solution.

“Thank God for Schervier Pavilion. I couldn’t see myself anywhere else,” he said.

From NY Route 17 East southbound
Head south on NY Route 17 East. Take Exit 124 for NY 17A toward Florida. At the traffic light, turn left on NY 17A East (Greenwich Avenue). Continue for 10 miles and turn right onto Van Duzer Place. Continue past the main hospital on your left. Schervier Pavilion will be straight ahead.

From New York Thruway/I- 87 northbound/southbound
Head north/south on the New York Thruway (Interstate 87) and take Exit 16 for NY 17 toward U.S. 6/Harriman. Merge onto NY Route 17 West. Take exit 126 for NY Route 94 toward Florida/Chester. At the traffic light, turn left onto NY Route 94 West (Summerville Way) and continue approximately 4.5 miles. Turn left onto NY Route 17A East (NY Route 94 West/North Main Street) and continue 5.5 miles. Turn right onto Van Duzer Place and continue past the main hospital on your left. Schervier Pavilion will be straight ahead.

From NJ Route 284 northbound
Head north on NJ Route 284 north. Turn right onto County Road 642 (Bassetts Bridge Road) and continue to the end of the road. Make a slight left onto Lake Wallkill Road, which will become County Road 88 (Liberty Corners Road) upon entering New York State. After approximately 3 miles, stay straight onto County Road 1 (Pine Island Turnpike). Continue 4 miles, then turn left to stay on County Road 1. Make the 2nd right turn to stay on County Road 1, which will become Grand Street after 2.5 miles. Turn left onto Van Duzer Place and follow the entrance to Schervier Pavilion on your left.

From NJ Route 94 northbound
Head north on NJ Route 94/County Road 515/Vernon Warwick Road. After crossing into New York State, remain on Route 94 for approximately 4.5 miles. The hospital will be on your left. St. Anthony Community Hospital will be on your left at the intersection of Grand Street and Main Street. Turn left at the first street after the hospital, Van Duzer Place. Schervier Pavilion will be straight ahead.

From Warwick Turnpike northbound
Head north on Warwick Turnpike. After entering New York State, continue on Warwick Turnpike (County Road 21) for approximately 2 miles until the road ends. Turn right onto NY Route 94 East and continue for 2.5 miles. St Anthony Community Hospital will be on your left at the intersection of Grand Street and Main Street. Turn left at the first street after the hospital, Van Duzer Place. Schervier Pavilion will be straight ahead.
